An exciting opportunity exists for the successful candidate to work within the Medicine Services Program by leading teams through transition and change while developing strategies to continually improve patient care. The candidate will continue to assess, refine and implement new patient care process flows as care and services offered evolve to meet the care needs of patients. The candidate will work with community partners and key stakeholders within the health system to optimize transitions through the care system.

As an integral member of the Medicine Services Program, the leader works in collaboration with the Director and Physician Leaders to provide leadership and direction for the Infectious Diseases Care Program and the Sexual Assault and Domestic Violence Program. The leader is responsible for the day to day operation and the coordination of access to care. The Coordinator utilizes clinical and leadership skills to ensure quality patient care through the planning, operational management, and coordination of human and physical resources. In addition, the incumbent will play a leadership role in developing care models in collaboration with community partners to ensure access to care is available in a timely and safe manner to vulnerable patient populations. The leader will also implement the site structure which will support the implementation of the Ottawa Model for Smoking Cessation at the St. Joseph's Hospital site within all outpatient and inpatient areas.

The position's responsibilities include managing and developing human resources; managing and administering operating budgets; developing, managing and leading projects that will result in optimized care delivery, efficiency and productivity within and across programs; developing, managing and implementing quality improvements initiatives to ensure continuous quality improvement throughout the program in alignment with corporate goals and strategies.

Essential Qualifications:

- Successful completion of a Bachelor of Science in Nursing or Allied Health Profession

- Current registration with relevant professional college

- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

- Ability and commitment to act as a supervisor under OHSA

- Minimum 3 years' experience in progressively more responsible clinical leadership role

- Solid knowledge and understanding of legislation related to Employment, People & Financial Management

- Demonstrated experience in program development, assessment and evaluation, project management and continuous quality improvement

- Demonstrated knowledge and/or previous experience of ambulatory care

- Demonstrated abilities in fiscal management and operational decision-making

- Demonstrated interest and experience in research and program evaluation
