Job Summary

Lifemark&'s Seniors Wellness program provides physiotherapy and related services in long-term care and retirement homes, and provides community-based programming to seniors with the goal to build strength and maintain mobility.

The Seniors Wellness program is seeking a Registered Physiotherapist to provide services in London.

The Physiotherapist will provide care to clients in a retirement home by conducting clinical assessments, developing individual treatment plans and delegating treatments to the Physiotherapy Assistant on-site. The Physiotherapist will also be responsible for conducting individual assessments through the retirement home&'s signature exercise program. The Physiotherapist must provide efficient service both professionally and ethically, delivered in accordance with the College of Physiotherapists of Ontario and company policies.

Required Competencies:

. Registered and in good standing with the College of Physiotherapists of Ontario.
. Resident Physiotherapists and/or those with Provisional Practice welcome to apply.
. ADP Authorizer for the assistive devices program is an asset (not required).
. Ensures that all documentation, charting, referrals, consent forms and other applicable documents are maintained in an organized and accessible format, in accordance with Lifemark&'s policies, College regulations and Ministry guidelines.
. Purchases & maintains malpractice insurance in keeping with College of Physiotherapy&'s guidelines (proof required upon hiring).
. Provides oversight and guidance to the PTA onsite.
. PT must be able to respond promptly to service request:
. Must be able to call a client within 24 hours of receiving a referral.
. Provide an assessment within 72 hours for urgent/short stay referrals.
. Provide an assessment within 7 business days for non-urgent clients.
. CPR/AED certification.
. Obtain the 2 step TB testing required upon hiring.

Duties and Responsibilities

. Supports carrying out the day to day tasks and goals as set by the rehab team.
. Enables client to live independently at home and in the community through providing support.
. Reinforces safety measures with client.
. Monitors triggers that are adverse to client&'s well-being.
. Observes and documents client conditions, reports unusual events or changes in the client&'s condition to the rehab team and/or nursing supervisor.
. Attends team meetings as requested.
. Participates in ongoing internal and/or external continuing education activities.
. Adheres to Bayshore Policies and Procedures.
. Participates in quality activities and continuous improvement initiatives in keeping with the company&'s Quality Management System.
. Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
. Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
. Completes other tasks as requested.

Experience

Minimum 1-3 years of Physiotherapy experience.

Education

. Registered Physiotherapist with College of Physiotherapists of Ontario.

Other Information

Ability to communicate verbally and in writing in either English. Demonstrated ability to follow written and verbal instructions as related to supportive care. Exceptional interpersonal skills and ability to work independently and as part of a team. Valid driver&'s license and reliable vehicle. Current criminal reference check and vulnerable sector screen.
